UPK Continual Registration

Registration For UPK

 

Registration for Universal Pre-Kindergarten is taken on a continual basis. To register your child for Universal Pre-K, call the Central Registration Office at 285-8890.  You will need to complete a New Entrant Form and an Emergency Contact Card.  In addition, you are required to present the following

information: 

-       A birth or baptismal certificate (original with seal).

-       Immunization records indicating specific dates signed by a physician or stamped by the physician or clinic

-       A record of a recent physical

-       Two proofs of residency (lease, deed, utility bill with parent/guardian’s full name and address)

-       Custody papers, if applicable
